I for myself, heirs, representatives or assigns, do hereby authorize and permit SoSu.TV and it’s client to use my likeness, voice, and image of self and my personal property in video / print communications. In granting this authority and permission, I for myself, heirs, representatives, or assigns do hereby release any and all claims arising out of said authorized use of my photograph, likeness, voice, and/or image as well as photography of my property or place of business.

 

SoSu.TV and it’s licensees are hereby released along with their successors, assigns, officers, agents, employees, and servants from any and all causes of action, claims, demands, costs, loss of service, expenses, compensation, medical expenses, compensations and all consequential damage on account of, or in any way arising out of said authorized use of my photograph, video, likeness, voice and/or image as referenced above.

 

I represent that I am 18 years of age or over. If I am under 18, my parent or guardian must sign below, agreeing to all the above terms for me.
